Visits must fit your plan’s monthly limits; unused visits are not assumed to carry over. Membership also provides ongoing physician access, but it will not reduce everyone’s spending.
You can also join Vanguard while keeping health insurance. Compare the membership fee with your actual out-of-pocket primary care costs and the value of direct physician access. Your insurance premium and charges for outside care do not disappear when you join.

Starting in 2026, qualifying direct primary care membership fees can be paid or reimbursed from an HSA. Eligibility depends on what the arrangement includes. Paying from an HSA and remaining eligible to make new HSA contributions are separate questions. Confirm your selected membership’s treatment before using HSA funds.

That is an effective after-tax cost of $113.24 per month; the membership charge remains $149.
Illustrative federal income-tax calculation: $1,788 x 24% = $429.12. Assumes the contribution is within your permitted limit and fully deductible at that rate. No state or payroll-tax saving is assumed. An HSA withdrawal is not a new deduction, and the same expense cannot be reimbursed twice. This is not an additional automatic discount to combine with the uninsured example.
Existing HSA funds may still pay qualified expenses even if you no longer qualify to contribute. Being uninsured generally prevents new HSA contributions; it does not by itself erase an existing HSA balance. The membership must still qualify, and the expense must meet HSA reimbursement rules.
